Tryout Dates U9-U18 Boys & Girls:
LOCATION: All tryouts will be conducted at Mountain Ridge Jr. High located in Highland, Utah. Please register at the fields and NOT at the Highland City Building. Please attend all tryout dates for your age.
BEFORE TRYOUTS:
1. Email a wallet size photo of your player(s) to Debbie Francis at: francis.p@francisperio.com Please include your child's name and birthdate.

Please Note*: Some U8 Players will play up one year in the U9 AA District Program or State X-League Program.
Also, The U in U8, U10, U13, etc stands for Under 8, Under 10, etc. as of August 1st, but they will be turning that age within the year.
Each Player should bring their own Ball, Cleats, Shin-Guards and Water Bottle to Tryouts.
PLEASE BRING THE FOLLOWING COMPLETED ITEMS TO TRYOUTS 30 MINUTES EARLY:
1ST DAY OF TRYOUTS:
1.
$5 Cash or Check (Payable to Champions FC) for Numbered Tryout & Practice T-shirt. Please Fill Out the
Equipment Order Form and Select your size for the Practice T-shirt Form at Tryouts.
2. Two Copies of Players Birth Certificate or Passport.
3. A completed UYSA Registration Form (Two Copies). This completed form will be kept in a book by each team admin and will be required along with birth certificate and medical release form.
4. Please note that you will be required to ALSO go online by the state and register here as well: www.uysa.champions.affinitysoccer.com Then Click on the gree "Registration" Tab at the top of the screen, then select "Player Registration".
4. A completed Tryout Waiver Form.
5. Player Action Form. Please Fill Out and Sign the "Player Information" Section.
6. A completed Parent Player Letter of Committment.
7. A completed Medical Release Form.
FINAL DAY OF TRYOUTS
1. Please Bring A Down Payment Check Payable to Champions FC for $350. Please make out a separate check for each individual child if you have more than one player in the club. This payment will help to fund UYSA Registration Fees For the Fall and Spring, Training Fees, Admin, Equipment and Uniforms. Remaining balance will be due at a later date. Total Fees For 1 Year: $552 (This includes $67 UYSA State Reg. Fee, $50 Referee Fees, $85 For New Uniform if needed). A final payment of $202 will be required no later than Feb 1st. *Please Note the District 5 Registration Fees may be around $78 per player who wishes to register as a AA District team and not X-League or AAA.
Tryouts will be overseen by David Januario, the club's technical director and other club trainers.
